Open, honest communication between you and your physician is integral to determining if medical cannabis is a viable option for your medical needs. If this is a a direction you’d like to head in, consult with your healthcare practitioner and consider the following in your conversation:
- Which conventional medications am I currently taking?
- What other therapies have I undergone in the past or am I currently undergoing?
- What side effects have I experienced from these courses of treatment?
- What level of success have these treatments resulted in thus far?
The more information you provide your healthcare practitioner with, the more they are able to join you in making an informed decision regarding the potential trial of medical cannabis.
We encourage all healthcare practitioners to familiarize themselves with the terminology, strains, and effects of medical cannabis, in order to help make the best possible recommendations for your specific conditions. The completion of the Medical Document acts as a prescription and provides authorization for your use of medical cannabis. The document must include recommended dosage and can have a duration of up to one year. Please send the completed Medical Document to CannTrust directly, via secure fax, or by regular mail.
Following the submission of the Medical Document by your healthcare practitioner, you, or a party designated to act on your behalf, are required to complete a Patient Registration Form (A, B, or C) in order to begin the use of medical cannabis for medical purposes.
FORM A) For applicants with a permanent residence, allowing for direct delivery
FORM B) For applicants with no permanent residence who will require shipment to an alternate residence, such as a shelter, hostel or similar institution.
FORM C) For delivery to your designated health care practitioner (a portion of this form must be completed by them).
You can order your medication as soon as we have your completed Medical Document and Registration Form. Once received, a customer care team member will call you to confirm your registration and help you place your first order.

The application process to qualify for our Patient Assistance Program is now quicker and easier than ever!
All we require is proof of financial status, via a Revenue Canada Notice of Assessment, to demonstrate eligibility.
We now also accept any provincial or federal attestation of disability, and any patient under the age of 18 will automatically qualify for the Patient Assistance Program.

Clients must confirm their eligibility on an annual basis. CannTrust reserves the right to request documentation of eligibility based on the eligibility criteria identified above and reserves the right to cease a client’s participation in CannTrust AccessTM Patient Assistance Program. CannTrust also reserves the right at any time to limit the size of this program and/or change or cancel this program at any time.

CannTrust offers hassle free access to medical cannabis for Canada‘s valued veterans and works hand in hand with Veterans Affairs to ensure seamless claim processing and prompt reimbursement. Once approved for assistance, veterans will quickly receive their medication.
CannTrust will submit medical cannabis claims if a veteran has:
- A Veterans Affairs client number beginning with the letter ‘K‘*
- A completed Medical Document filled out by a healthcare practitioner
- Confirmation of registration with CannTrust
- A letter or documentation of your specific medical condition(s)
